First make sure that in your steam settings the most appropriate controller is selected. Then, in steam, in the Dark Souls 3 controller configuration choose a configuration that you think may work. Little over 200 hours in, and this is the first time I'm having any problems with the game. Anyone have any ideas? My fix: Nvidia Control Panel -> Manage 3D settings -> OpenGL rendering GPU set to force on the Graphics Card, (which in my case is a gtx 970).
